Box 1 Complete: Player name and Club 

Box 2: IF YOUR CHILD HAS ANY MEDICAL OR PHYSICAL CONDITIONS THAT MAY LIMIT THEIR ABILITY TO PARTICIPATE: Doctor must complete: Child's Name, explain their condition, and have their physician sign and date.

** If you are not aware of any medical or physical condition(s) of your child that would limit him/her participation in the VALLEY YOUTH CONFERENCE FOOTBALL program, PLEASE LEAVE THIS SECTION BLANK.
